United Lincolnshire Hospitals NHS Trust’s pharmacies at Lincoln County Hospital and Pilgrim Hospital, Boston provide services to inpatients within the hospitals. Outpatient services are provided by Lloyds Pharmacy. The pharmacies at Grantham and District Hospital and County Hospital, Louth provide both inpatient and outpatient services.
The clinical pharmacy teams provide a clinical pharmacy service to individual patients on the wards by checking patients prescriptions to ensure the right medicine is being given and regular monitoring of the patient’s condition. Medicine safety is of paramount importance. We can educate patients on what medicines they need to take, when to take them, what they are being used for and possible side effects.
The medicines information section of the pharmacy team supports the safe and effective use of medicines by providing evidence-based information and advice on the management of medicines within the Trust and for the care of individual patients.
